•At any City of Edmonton leisure, sports & fitness participating facilities.

& s8 }. N7 W, A) R5 y: E7 F Monday-Friday except holidays, 8:30am-4:30pm.
•By calling 780-496-4918 to have one mailed to you.
: l. Q& s3 o" h5 j/ V  U•For individuals under custodial care, please also submit the address verification form.
•For individuals under government care, please also submit the consent for release of information form.
