# W, _( v" v. |9 S6 ~$ `! Y, NThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.: ~& E% G9 ^1 ]

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. 0 M7 h: V r5 S! W; R
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. # J; l* Z4 i0 K$ g$ e
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.3 a+ h' ]( _& F* K5 T
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

• Should have the ability to act responsibly, accountable, excellent problem solving/critical thinking skills.

Qualifications/Experience" I- Q3 K5 w" K
• Baccalaureate degree in Nursing preferred! g' K9 Y# S2 ?. s k4 p7 w" f
• Preference given to those with at least 5 years nursing experience in primary care setting.6 C. u+ t9 {+ K# }2 ?2 H$ [
• Current CARNA Registration7 f9 N! B2 B9 ~# }8 s
• Current CPR – Healthcare Provider" ^) y5 l) z# r& ~- g6 ~
• Must have own vehicle and valid driver’s license- J4 z( G G) ^$ m
• Knowledge of, and ability to use, electronic medical records$ z# n% A- z( v' o7 n
