Primary purpose: The Social Worker works in partnership with settlement workers or
' y' s- t! i I( M, fcommunity cultural brokers from New Canadian Health Network partner organizations to & G+ k+ _3 b5 `% ~/ T9 \9 y( j
provide supplemental support to newcomers who have complex needs. ; P( V2 Y3 Z/ p- v1 ^3 f* v
1 W; v+ G2 ~" n/ X Nature of Work: The Social Worker will report to the manager of mental health services. 0 p2 b0 m. }5 v
The position builds working relationships with New Canadian Health Network partners’ staff,
# Z, i( }: M# L d. W. `+ g% mnewcomers accessing these services and other institutions and service providers. The Social
% ^# n& E4 ]: Q0 v! z1 J/ i) NWorker will assist in case planning, conferencing and providing support and advocacy as
4 z/ A; f% c% R: Z" Z9 K) r9 tneeded. As required, the Social Worker will assist in accessing community supports for
" j# {* W6 K0 B& {2 x. k1 J) }newcomers and their families. The Social Worker provides holistic and flexible initial
7 f) F/ u+ f2 B" @4 bassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Developing of social networks and community supports for newcomer families. - e) p+ C/ Y: {: O6 i( l, t
Provide intake, needs assessment, referral, case plans and action plans to support 8 t U$ ]$ Y7 n* w" M

Work in partnership with settlement workers or cultural brokers to provide supplemental

Treat all clients with dignity and patience in order to listen deeply and give thoughtful

Qualification
! }0 T1 \6 q4 a* Q2 ^/ M( G- p& Z; ` 5 m7 Z4 X5 n$ u s
Relevant undergraduate degree or diploma, or equivalent combination of education and
! ^- l- M; Q5 q+ c5 u$ iexperience in Social Work or related human service fields.
' h! n. `7 B2 X+ Y2 X Extensive knowledge of community resources, including in depth understanding of family
% @' ^9 ]1 z( L0 N3 Asupport needs 0 L. f1 q, l* x7 T
Experience/facility with MS Office computer applications
/ w9 Z, X4 m9 s% x% n2 Q1 X' G Proficiency in both official languages is an asset, as is fluency in other languages
