Primary purpose: The Social Worker works in partnership with settlement workers or

provide supplemental support to newcomers who have complex needs.

Nature of Work: The Social Worker will report to the manager of mental health services. " f/ N: y) v; \; K) X% P& A3 i! y
The position builds working relationships with New Canadian Health Network partners’ staff, 3 j2 P# E) N$ P0 @
newcomers accessing these services and other institutions and service providers. The Social
6 M4 N, P- L0 ?Worker will assist in case planning, conferencing and providing support and advocacy as
1 v( Q* `6 I+ ?2 pneeded. As required, the Social Worker will assist in accessing community supports for ! }# G2 D% A& u% ^; W8 Y' E/ q& O
newcomers and their families. The Social Worker provides holistic and flexible initial
& m& c, ]4 p1 Q5 | xassessment of life circumstances, needs, goals and aspirations and adheres to the client-+ ~' F, R5 F: u& b$ C2 k

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental 6 j$ M' y- `8 s' G) Y

Advocate on newcomers’ behalf to programs, institutions and service providers.

Extensive knowledge of community resources, including in depth understanding of family
' c, }& P- d; Usupport needs ( z, u4 S% `9 }6 ^/ z$ N
Experience/facility with MS Office computer applications ( Q* v+ t/ l- Y# L
Proficiency in both official languages is an asset, as is fluency in other languages $ K0 x' d& A" f7 X+ o
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
