Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
$ w( ^4 B' V, j+ Q2 k" C) ]% mTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
# T4 B) S6 V( Z$ O4 t; y% tPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
; y" w6 K0 c7 n7 S' i: i. C8 ?2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
$ w2 s; H. c: H1 e* e3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
0 o0 \2 `3 w9 I( p2 P5. Children are not required to be present at the Registration Day
& U) N6 W# c7 y7 q1 ]6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
