8 ?' m5 F' ?; n5 _Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
9 W! `% R" O' uPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
9 i8 {4 {  g: Q/ S  UFees        Ranges from $10 to $35 per week
       
Remarks:
* m! T- a" J7 J" Q9 |$ M2 D) \1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
3 @) }9 J0 d& s4. Information on Child’s swimming level when registering for swimming classes
3 }" w3 q  C" `' f5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
