Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)

Place        ASSIST Community Services Centre (9653-105A Ave)
1 d  V& y- \7 ?2 Q( x- YPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

% m! t" x  S6 v, u) Z2 E/ @# j. [1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
5 A' m0 p* G) g- |4 V3 J  s. r2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
- ?3 c; W4 x  u0 J! ~% _3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
7 [. t$ j" L1 A5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
