Abstract: (2) unique/new employment opportunities in the Transitions Program. As a Case Manager or Community Outreach Specialist you will support clients as they move from the street to living healthier/safer lifestyles. Program services are delivered one-to-one in the areas of life management, crises intervention & management, decision making, community resourcing and employment related services/supports. / C4 x+ Z7 n! f. j& m

Key Qualifications for both positions: post secondary degree with 2 to 4 years experience or equivalent.

(a) Case Manager in DECSA's Job Placement Program, you will support clients with documented disabilities on a one-to-one basis with job placement.
