1.Print the Leisure Access Program Application Form.The form is also available:

•14th floor CN Tower, 10004-104 Ave.
Monday-Friday except holidays, 8:30am-4:30pm.
( ]; h& l7 q0 G/ B( O0 b•By calling 780-496-4918 to have one mailed to you.
* P: f( }( T5 X5 g7 ~•For individuals under custodial care, please also submit the address verification form.
