Nature of Work: The Social Worker will report to the manager of mental health services.
% ?2 L" n4 |4 a" hThe position builds working relationships with New Canadian Health Network partners’ staff,
$ s" G4 C$ v: d5 S. }) q0 D; D/ v* Dnewcomers accessing these services and other institutions and service providers. The Social
/ Y% E9 \" @ E. D1 e2 QWorker will assist in case planning, conferencing and providing support and advocacy as 7 t- d& }: [" g& u+ T
needed. As required, the Social Worker will assist in accessing community supports for
5 q) O) ~$ y$ }0 S8 X6 h* I1 Nnewcomers and their families. The Social Worker provides holistic and flexible initial
5 K. {+ z% ^) ]) K# V+ A; ^, aassessment of life circumstances, needs, goals and aspirations and adheres to the client-

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental $ E! I& d- _# G9 N, u$ X7 U, j: M7 o

Advocate on newcomers’ behalf to programs, institutions and service providers.

support needs
