Primary purpose: The Social Worker works in partnership with settlement workers or

5 x4 q# z p' H0 b, T: K$ _6 I) n Nature of Work: The Social Worker will report to the manager of mental health services.
$ h: ~) b& S3 C, _, O* _1 \The position builds working relationships with New Canadian Health Network partners’ staff, 5 t3 E7 H, j4 a' z
newcomers accessing these services and other institutions and service providers. The Social
0 T K- B3 A1 P; ^/ B- K% u9 BWorker will assist in case planning, conferencing and providing support and advocacy as $ `! O9 g. ]0 m& V% j) i- V
needed. As required, the Social Worker will assist in accessing community supports for 8 ^# E: F# }" ^$ x3 M( s
newcomers and their families. The Social Worker provides holistic and flexible initial
" E& R9 K9 G9 u( h: {" bassessment of life circumstances, needs, goals and aspirations and adheres to the client-" @ J2 r+ g: m8 T' Y

Developing of social networks and community supports for newcomer families.

# V, v0 N1 N5 x6 y/ ynewcomers in settling and fully participating in community.

case plan support for clients who have complex needs.

Qualification
" x/ b @1 P8 ?1 S
* q- s+ Z. A+ g' G S, W5 U5 _ Relevant undergraduate degree or diploma, or equivalent combination of education and 6 v$ k+ v( e! K) V. l
experience in Social Work or related human service fields.
* y- X( l) w7 o& G: n4 P0 g Extensive knowledge of community resources, including in depth understanding of family " R/ B3 R0 B6 V9 s: `8 E& n. \1 X' `

: P- B: ?% K( B! Z Experience/facility with MS Office computer applications
1 Z7 }: T6 x+ v7 {/ q Proficiency in both official languages is an asset, as is fluency in other languages
