Primary purpose: The Social Worker works in partnership with settlement workers or - T7 u, h, E; H: n4 h# p
community cultural brokers from New Canadian Health Network partner organizations to

8 `8 r# Y% Y9 \" a4 \$ j Nature of Work: The Social Worker will report to the manager of mental health services. ' e. l/ J, `) A, S+ { {6 D
The position builds working relationships with New Canadian Health Network partners’ staff,
; J8 l d" d9 _: pnewcomers accessing these services and other institutions and service providers. The Social m: [2 X) L c
Worker will assist in case planning, conferencing and providing support and advocacy as ; m. t. T% g% E
needed. As required, the Social Worker will assist in accessing community supports for " p1 {4 [" M" U0 `. \: c# b" v$ V& A! @) r
newcomers and their families. The Social Worker provides holistic and flexible initial
- u+ ~; o( t- z7 Kassessment of life circumstances, needs, goals and aspirations and adheres to the client-
9 w$ E4 [% ^" S3 Y7 b7 P/ Ocentered delivery model.

Developing of social networks and community supports for newcomer families. 9 N1 q* f8 k. }( ~+ q
Provide intake, needs assessment, referral, case plans and action plans to support - W: s, G4 q! v8 l' Y2 T6 T
newcomers in settling and fully participating in community.

Advocate on newcomers’ behalf to programs, institutions and service providers. + _$ q2 j$ _+ ]$ t$ U; w
Treat all clients with dignity and patience in order to listen deeply and give thoughtful 8 Y- b( |$ z' T, y" i# r

Qualification
3 ~9 M9 ^4 E# \" }+ X' N ) X, z) C3 y: a% x4 f5 g
Relevant undergraduate degree or diploma, or equivalent combination of education and
+ q+ @. z) _& c+ k) s9 Nexperience in Social Work or related human service fields.
2 Y/ P _1 X+ W4 p9 ^5 p9 b Extensive knowledge of community resources, including in depth understanding of family 8 e0 \' [. X' ]' j r! ^+ S
support needs , y( L0 v( }9 P
Experience/facility with MS Office computer applications
+ z- t, M, g( d Proficiency in both official languages is an asset, as is fluency in other languages
+ g* p: w' T$ q6 i/ E Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate $ f) ]3 n3 S' f0 @/ R
