Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
3 v6 }( ?6 ]  P' ]9 C! {Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

8 a$ j8 j: e6 G' S1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
. u" z& T- C8 x; e) a+ ]1 y) I6 W3 t2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
8 W1 Y) u# n$ {6 Y1 v3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
" r. s8 u- i- j0 D5. Children are not required to be present at the Registration Day
6. Registrations will be accepted on a first-come, first-served basis

4 ~2 M' ?0 I" c+ y- |3 FWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
