Target:        Low income families with children/youth aged 3-17 residing in Edmonton
! j4 ~/ _! y/ @, N  NDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
1 l3 E/ }; N; n( z% S: `( `4 z7 U- jPlace        ASSIST Community Services Centre (9653-105A Ave)
2 o( ~) P1 P7 f' G% N  }1 `# T9 VPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
6 R: V& }  k% ^) DFees        Ranges from $10 to $35 per week
; O& }* I) j5 x9 J, c8 N# P' D- ^       
- _8 p$ ?; r9 k$ b! b7 y" J& g+ JRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
3 W0 D1 B$ J9 S$ l4 `$ o( W2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
! Q( H- s6 z( |, C& p3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
8 R6 w! c$ ?7 ~8 k; E/ \, {5. Children are not required to be present at the Registration Day
) P) T; X9 [3 v$ X4 v6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
