Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
4 K: y& H' @! Z% P5 FTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
$ o1 W+ n3 a0 XPrograms         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
; m0 U. I) [  S0 P2 YFees        Ranges from $10 to $35 per week

% a7 H4 V8 }6 \2 ~" G9 d$ gRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
5 s: {7 s* R/ ?; S2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
# W$ L- ?7 n5 k& s4 H( a8 [+ C5. Children are not required to be present at the Registration Day
% M( E$ R1 o+ [/ L1 U2 o& L. ?. a6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
