When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic 8 C' k) O$ A( E* m4 m2 mcondition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and1 P0 ^4 B6 q; F+ t4 ]
• there have been no new symptoms or change in symptoms; and/or: \8 A. G7 |- O$ @1 A; i$ Z
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy. # I7 F& \- o; l$ Y& Y0 WIf “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
