When the option is included or purchased, exclusion 1 in the policy is modified such that, after any applicable waiting period, a stable chronic9 ]) v) q) m( a9 f/ \
condition (as defined) is covered under the policy.
“Stable Chronic Condition” means: a condition which is under treatment and which has been controlled by consistent use of medication prescribed by a “physician”; and5 S# f7 J* l4 W
• there have been no new symptoms or change in symptoms; and/or, c, {8 v8 ?9 a. q: J% R
• there has been no “hospitalization”or change in treatment, medication or dosage in the 180 days prior to the “effective date” of this policy.* g' L/ L: j% ?: d
If “your” “physician” has determined that “your” health condition has improved and changes “your” treatment or medication or its dosages due to “your” improved health condition, this does not constitute a change.
