POSITION SUMMARY

9 b- T; L6 E; P- V/ w4 E9 W' T" UThe Primary Care Nurse will provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The Nurse will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physician and multi-disciplinary team to develop and implement care plans and to provide follow-up with the patient. The Nurse will work to full scope of practice within the primary care setting; some duties will vary depending on the clinic and patient population.% e- D% b9 N1 f2 x6 f5 W
) i1 I+ F0 d* W Y( H! y: qThe Primary Care Nurse will work with Family Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

• To work collaboratively within a family practice environment to enhance the delivery of primary care services.

• According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up.

• To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care. , O9 E0 G' q+ u, r. m
• To develop long-term, trusting relationships with both patients and peers in providing primary care services. $ ]& s% r$ j9 O% k, R
• To assist in the delivery of disease management and health promotion programs according to clinical practice guidelines.

• To coordinate with Alberta Health Services and ensure that relevant regional programming is effectively used.+ L8 r z# ?5 v( V* _: @
• To act as a resource to other multidisciplinary team members, physicians, and community resources.

Qualifications/Experience
" p6 H3 z# H( K3 O& [2 D• Baccalaureate degree in Nursing preferred
7 P0 p1 R8 M1 |• Preference given to those with at least 5 years nursing experience in primary care setting.
) U2 y$ R( a$ |9 Z• Current CARNA Registration
7 X2 c" O1 d+ r$ N, P. L• Current CPR – Healthcare Provider: O1 Z2 f+ W$ I9 X/ O
• Must have own vehicle and valid driver’s license
