The position builds working relationships with New Canadian Health Network partners’ staff, ) L" Z& H' |3 x. ?0 A
newcomers accessing these services and other institutions and service providers. The Social
4 g+ N) D1 |% K! C1 k5 mWorker will assist in case planning, conferencing and providing support and advocacy as 7 z" k" b% V6 m' D8 F% N7 Q& @% N
needed. As required, the Social Worker will assist in accessing community supports for
: _+ x+ _. X3 K2 o9 x9 W3 ?5 J7 Gnewcomers and their families. The Social Worker provides holistic and flexible initial " O& [" q7 g: c1 Q0 O$ C
assessment of life circumstances, needs, goals and aspirations and adheres to the client-) Q c. C+ E6 G+ _+ B: v) k

Relevant undergraduate degree or diploma, or equivalent combination of education and - i: R) K/ B* }
experience in Social Work or related human service fields. ! Z0 D! ?+ L; S! c
Extensive knowledge of community resources, including in depth understanding of family & w% q4 X3 k5 e# m. D& h5 w
support needs / V5 o6 q( m% o! m! K+ i
Experience/facility with MS Office computer applications
