3 P, X& q' R1 } Nature of Work: The Social Worker will report to the manager of mental health services.
8 g9 e2 R8 ?1 W, C8 f* kThe position builds working relationships with New Canadian Health Network partners’ staff, 1 Y' J0 n. e7 K+ I
newcomers accessing these services and other institutions and service providers. The Social
2 Z! Z2 j! C/ `( v9 {3 c. L( _6 P8 SWorker will assist in case planning, conferencing and providing support and advocacy as
% E& M" p4 K, K8 y" Q3 bneeded. As required, the Social Worker will assist in accessing community supports for ! _7 J4 ]# h2 w+ H* _
newcomers and their families. The Social Worker provides holistic and flexible initial
2 V# m4 r5 o4 q2 Cassessment of life circumstances, needs, goals and aspirations and adheres to the client-0 r5 m8 P7 x3 h, q) ~% X9 b
centered delivery model.

Developing of social networks and community supports for newcomer families.
% }4 p) v) K+ l Provide intake, needs assessment, referral, case plans and action plans to support ( q/ H7 _) \/ M: j/ X4 [+ B
newcomers in settling and fully participating in community. ; ]( }- B+ o8 ~& ]! V
Work in partnership with settlement workers or cultural brokers to provide supplemental 6 _+ O3 d+ x: M: j9 r- B
case plan support for clients who have complex needs. ' e# U# G3 {8 X5 r; y
Advocate on newcomers’ behalf to programs, institutions and service providers.

Qualification
/ j! U) I4 |' t% y. z" M0 X
* C" P8 Q2 b) b4 T, Q: l1 F% |6 y- f; F# f Relevant undergraduate degree or diploma, or equivalent combination of education and . C- ^% L8 d3 ]7 Y" Y# f8 l- u
experience in Social Work or related human service fields.
$ r u1 V+ i7 d/ a. J% p Extensive knowledge of community resources, including in depth understanding of family
" c1 k" I! R0 Z" W) ~7 X2 hsupport needs ; W1 ]& v* Q! c
Experience/facility with MS Office computer applications - R& y* n. a9 G
Proficiency in both official languages is an asset, as is fluency in other languages ; F! u2 O, o( @2 B1 i9 l* w7 q8 O
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate 7 b1 | R; V" X0 D0 B3 J
insurance coverage for work
