Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
4 }. B( v9 k* a% E: z' ]8 P( V$ c, zPlace        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
9 C! C* ?# g4 Q8 q9 w) hFees        Ranges from $10 to $35 per week

1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
8 Y. n$ S; Q: p) N  @2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
$ V  L- k4 e1 Z7 J3 C4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
. q( |9 z+ M7 u% |8 ~2 n7 T; W6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications
$ w" P+ J" z6 U, e& i7 X1 E
4 D8 R! \* j4 l; L+ U/ TWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
