其中包含了Initial card, replacement or renewal。你的应该是属于renewal类别。
在里面有几个表格

2.Supplementary Identification [IMM 5455]  (PDF, 560 KB) January 2012
3.Photograph Specifications for Permanent Resident Cards (PDF, 989 KB)

5.Instruction Guide [IMM 5445]
( v' w* Z+ y3 a另外，对换发的PR卡还有照片的要求，要到有资格的照相馆照相。照相的要求在Photograph Specifications for Permanent resident Cards (PDF, 1.05 MB).
在London Drugs或Sears 的照相馆，都是有资格照PR卡相片的。Sears照相馆比较专业，但价格贵。
