Primary purpose: The Social Worker works in partnership with settlement workers or % J5 t! g' m; z3 U5 B+ s1 V& G
community cultural brokers from New Canadian Health Network partner organizations to
5 S; Y% V4 _ m5 n2 N8 pprovide supplemental support to newcomers who have complex needs. 9 K! e) P# q" p! ]0 b9 Y
9 ^/ v v, F5 P+ m Nature of Work: The Social Worker will report to the manager of mental health services.
9 z2 a4 m, H3 A( EThe position builds working relationships with New Canadian Health Network partners’ staff,
) Q" q4 L: ^5 Hnewcomers accessing these services and other institutions and service providers. The Social
3 b- e5 g% v. n! z+ n1 M) WWorker will assist in case planning, conferencing and providing support and advocacy as 6 {% c c( Q) _ |: U5 n
needed. As required, the Social Worker will assist in accessing community supports for ( p. @3 a, ?7 ^- i0 V
newcomers and their families. The Social Worker provides holistic and flexible initial # O' ?( E( Y; d1 ?
assessment of life circumstances, needs, goals and aspirations and adheres to the client-

Providing holistic information or advocacy services for immigrants and refugees served.

Provide intake, needs assessment, referral, case plans and action plans to support

Work in partnership with settlement workers or cultural brokers to provide supplemental , l' B; M. m: p1 w1 X* F4 n5 G$ s6 U4 i

Advocate on newcomers’ behalf to programs, institutions and service providers. * c- a6 E3 |! y9 w& @" p

$ X1 W. N; m8 F3 | V. u8 @5 N Relevant undergraduate degree or diploma, or equivalent combination of education and 5 X8 p$ `" y, t! b. J9 Q
experience in Social Work or related human service fields. . M8 T$ D. m) m( _. @
Extensive knowledge of community resources, including in depth understanding of family
' O( P8 z1 ~8 D$ L# ?% Esupport needs , c P5 ^ s; T& {: |; c
Experience/facility with MS Office computer applications 6 c4 f4 _9 ?3 k% c
Proficiency in both official languages is an asset, as is fluency in other languages 0 I1 S, A" Q/ q0 s" T8 q A
Possess a valid class 5 driver’s license and own a reliable vehicle with appropriate
/ N5 t" w5 _0 M+ X. t+ x H0 `insurance coverage for work
