Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
/ s; ~& V9 y( G8 r; ~Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
( }, K1 b4 k4 ~Fees        Ranges from $10 to $35 per week

Remarks:
# {0 s" e! j+ b/ A0 ~3 Y1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
' ?4 J9 T2 J5 @! B- i6 ?2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
6 h" c2 y( Q/ }" ^5. Children are not required to be present at the Registration Day
# ?# H  {" c6 U2 @) `9 b6. Registrations will be accepted on a first-come, first-served basis

Fee Reduction Pass Applications

We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
