- i( k, [' r3 I% C' zTarget:        Low income families with children/youth aged 3-17 residing in Edmonton
  o$ A' [: J8 xDate        May 14, 2005 (Saturday)
Time        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
4 K4 B4 j7 j. c% q& B8 R' Q0 aFees        Ranges from $10 to $35 per week
       
Remarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
% z$ ?# G- y1 u/ g4 k; F- r3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day

Fee Reduction Pass Applications

3 A; s* C( O% l- T# `8 _We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
